Royalty Pharma Acquires Interest in ATTR-CM Therapy for up to $425 million

Royalty Pharma said that it has acquired a portion of Neurimmune’s royalty interest in AstraZeneca’s cliramitug for up to $425 million.

The transaction includes $125 million upfront, with additional payments tied to near-term funding and clinical and regulatory milestones. In exchange, Royalty Pharma will receive a 3 percent to 4 percent royalty on global net sales of the therapy.

Cliramitug is an experimental monoclonal antibody testing designed to deplete transthyretin (TTR) amyloid fibrils in patients with transthyretin amyloidosis with cardiomyopathy (ATTR-CM), a progressive and often fatal condition caused by the buildup of misfolded proteins in the heart. Unlike currently approved therapies that slow further amyloid accumulation, cliramitug seeks to remove existing deposits, an approach that could potentially modify disease progression more directly.

The therapy is currently being evaluated in a phase 3 trial, with results expected in 2028. AstraZeneca has previously projected peak annual sales for cliramitug between $3 billion and $5 billion, underscoring the commercial opportunity in a market that surpassed $7 billion in 2025 and continues to grow rapidly as diagnosis rates improve.

“Cliramitug combines a differentiated scientific approach with promising clinical data and addresses a rapidly growing market with significant unmet need,” said Royalty Pharma CEO Pablo Legorreta. “This therapy further bolsters our development-stage pipeline, and we believe it will become a valuable contributor to our portfolio over the long term.”

For Neurimmune, the agreement provides non-dilutive capital to support its internal pipeline while allowing the company to retain the majority of its royalty and milestone rights.

Under the terms of the agreement, Royalty Pharma will make an additional $125 million payment in the first quarter of 2027, with the remaining $175 million contingent on development and regulatory milestones.
